Pre-Registration / Item Order Form

  Use this form to pay online, it's as easy as sending an email.  Once you fill out and submit this form you will receive an invoice via email from PayPal, usually within 24 hours, just follow the instructions in the email and your all set.  Packages will be available at the registration booth on October 15 & 16 at 1:00pm, just bring a copy of the invoice receipt you receive from PayPal to claim your merchandise.

  Please specify the T-Shirt sizes by placing the number of shirts in the appropriate box, please ensure the numbers in the boxes below match the number requested above or your order may not be filled.
